Registration Requirements

Please complete the following forms for registration:

• Registration Form
• Medical History Form (The Physical Examination Form must be completed by your child’s physician)
• Home Language Survey
• Student Residency Declaration

Additionally, please provide copies of the following documents:

• Your child’s Birth Certificate
• Your child’s Immunization Record and Physical Examination (within one year)

Proof of Residency:

One of the following is required:

• Deed/Closing Statement
• Current Property Tax Bill
• Current Lease/Rental Agreement

Two/Three of the following are required:

• Current Electric/Gas Bill
• Current Cable Bill
• Current Water/Sewer Bill
• Current Trash Bill
• Auto/Homeowner Insurance
• Vehicle Registration

If only two of the above provided, provide one of the below:

• Pay stub from employer
• Current cell phone bill
• Current credit card bill
• Current bank statement
• Government Issued Identification (ex; military ID, not drivers license)

Parent(s)/Guardian(s) must also provide a driver’s license with a valid Sussex/Wantage address.
