Dear Parents/Guardians:

In accordance with the regulations of the State of New Jersey, all children entering sixth grade or reaching the age of 11 require meningococcal and Tdap immunizations.  The Public Health Department will not allow your child to begin the next School Year without proper documentation of these immunizations. 

Physician documentation of these vaccines must be submitted to the certified school nurse prior to entrance into school. There is no grace period and there will be no exceptions. All 6^th graders must be in compliance.

The Sussex-Wantage School Nurses will be updating student records on a continual basis. Any time your child receives these mandated vaccines please forward verification to the Health Office of your child’s school. If you have any questions, please contact your school nurse.
